Who can Enroll?

📌 Eligibility:

• The principal applicant must be 18 to 65 years old.
• Dependents may be added based on the applicant’s marital status.
• Hierarchy ruling must be followed when enrolling dependents.

📌 Qualified Dependents (Following Hierarchy Ruling):

✔️ For Single Applicants:

1️⃣ Parents (up to 60 years old) must be enrolled first

2️⃣ Siblings (30 days t0 21 years old) may be enrolled after parents

✔️ For Married Applicants:

1️⃣ Legal spouse (up to 60 years old) must be enrolled first

2️⃣ Children (30 days to 21 years old) may be enrolled after the spouse

✔️ For Single Parents:

• Children (30 days to 21 years old)

Mode of Payment 🙂

I. Cash – Straight Payment only

    A. Online
– Fund transfer via bank or gcash
– Straight payment

    B. Bank Deposit
– Any MediCard bank account
– Straight payment

    C. MediCard Standing Clinics
– Straight payment

 II. Credit Card – Straight payment or Installment
A. Online
– BDO only allowed for installment zero interest.
– Other credit card would be straight payment

B. MediCard Standing Clinics
– BDO & BPI allowed for installment zero interest.
– Other credit card would be straight payment

Note: General Ruling

✔️ If total premium is 25k and above can be allowed for installment via BDO and BPI credit card payable in 6 months.

✔️ If total premium is 50k and above can be allowed for installment via BDO and BPI credit card payable in 12 months.

To apply MediCard Plans, need to fill out the form per enrollee and 1 valid

How to Apply for a MediCard Plan

1️⃣ Select a Plan – Choose the health plan that best fits your needs.
2️⃣ Consult with a MediCard Officer – A dedicated officer will guide you through the benefits and coverage.
3️⃣ Complete the Application – Fill out the required form and submit a valid ID.
4️⃣ Application Validation – Your application will be reviewed and verified by a MediCard Officer.
5️⃣ Payment Processsing – Once validated, you may proceed with the payment.
6️⃣ Applications Confirmation – Upon successful payment,your application will be processed
accordingly.

Medical Evaluation: 6-Step Process

1️⃣ Complete the Application Form – fill out the required form and submit it to us.
2️⃣ Receive the Medical Evaluation Form – We will provide the necessary medical evaluation documents.
3️⃣ Schedule Your Evaluation – We will arrange an appointment at the nearest MediCard Standing Clinic.
4️⃣ Undergo Medical Assessment – Attend your scheduled consultayion and laboratory procedures at the MediCard Clinic.
5️⃣ Await Evaluation Results – Your medical assessment will be reviewed.
6️⃣ Finalize Application & Payment – Once approved, you may proceed with the application and payment process.

✅ 1. Request at the Hospital HMO Department

• Visit any accredited hospital’s HMO department and request LOA approval.

✅ 2. Use the MediCard Go App 📱

• Download the MediCard Go App from the Google Play Store or Apple App Store to request an LOA easily.

✅ 3. Email Your Request ✉
